      [摘要] 目的 探讨宏基因组二代测序(mNGS)在儿童中枢神经系统感染中的价值。 方法 选择2023年1月至2024年5月首都医科大学附属北京儿童医院黑龙江医院收治的45例疑似中枢神经系统感染的患儿,收集脑脊液6 ml,分别进行mNGS与常规检测后进行比较。 结果 经过mNGS技术检测,成功识别出共计26例可疑病原体,同时,脑脊液微生物培养技术检测出3例疑似存在的可疑病原体。在诊断中枢神经系统感染性疾病方面,mNGS组的灵敏度、准确度以及阴性预测值均高于微生物培养组,差异有统计学意义(P < 0.05),而mNGS组的特异度及阳性预测值方面均相近于微生物培养组,差异无统计学意义(P > 0.05)。结论 在评估儿童中枢神经系统感染的诊断时,相较于脑脊液微生物培养检查,mNGS的优势显著,但仍应结合多种检验方法综合判断,以达到准确判断的目的。

      [Abstract] Objective To investigate the value of metagenomics next-generation sequencing in children with central nervous system infection. Methods A total of 45 children suspected of central nervous system infection admitted to and treated in Heilongjiang Hospital of Beijing Children’s Hospital Affiliated to Capital Medical University from January 2023 to May 2024 were selected, and 6ml of cerebrospinal fluid was collected, and then the metagenomic next-generation sequencing (mNGS) was compared with conventional detection, respectively. Results A total of 26 suspected pathogens were successfully identified by mNGS technology, and 3 suspected pathogens were detected by cerebrospinal fluid microbial culture technology. In the diagnosis of infectious diseases of central nervous system, the sensitivity, accuracy and negative predictive value of the mNGS group were all higher than those of the microbial culture group, with statistically significant differences (P < 0.05). While the specificity and positive predictive value of the mNGS group were all similar to those of the microbial culture group, without statistically significant difference (P > 0.05). Conclusion When evaluating the diagnosis of children’s central nervous system infection, mNGS has obvious advantages compared with cerebrospinal fluid microbial culture examination, but it should still be combined with a variety of examination methods to achieve the objective of accurate judgment. mNGS has obvious advantages compared with cerebrospinal fluid microbial culture examination, but it should still be combined with a variety of examination methods to achieve the objective of accurate judgment.
