| [摘要] 围绝经期及绝经后女性骨质疏松症的患病率高、知晓率低、诊断率低、治疗率低，一旦发生骨折治疗费用高、致残率高、病死率高，严重影响女性身心健康、降低生活质量。中医“治未病”理论包含的“未病先防、既病防变和瘥后防复”三层涵义，契合健康管理的三级预防，即病因预防，早发现、早诊断、早治疗的三早预防，疾病管理临床预防。对于该类人群，在现代医学模式及中医思想指导下，应用医学和管理学知识与移动互联网等现代信息化技术手段，积极开展“互联网+”的中医健康管理服务模式，提供个性化、专业化、智能化的中医健康管理服务。利用杭州市围绝经期妇女保健中心、杭州市更年期保健规范化专科门诊管理平台、杭州市妇幼保健专科联盟，通过互联网医院女性更年期关怀服务包,建立以骨质疏松性骨折的危险分层为基础，以三级医疗机构的分工合作为主体的围绝经期及绝经后骨质疏松症的三级防控体系,实现慢病的全程长期有效管理。 |

| [Abstract] The incidence of osteoporosis in perimenopausal and postmenopausal women is high, while the awareness rate, diagnosis rate, and treatment rate of osteoporosis are low. Once fractures occur, the cost of treatment, the disability rate, and the mortality rate are high. This seriously affects the physical and mental health of women and reduces their quality of life. The theory of "preventive treatment of disease" in traditional Chinese medicine includes three layers of meaning, "preventing diseases before they occur, preventing existing diseases from changing, and preventing recurrence after they occur", which is exactly in line with the three-tier prevention of health management, i.e., the prevention of causes, prevention of the disease at an early stage of early detection, diagnosis, and treatment, and prevention of disease in the clinical management setting. For such a population, under the guidance of modern medical model and traditional Chinese medicine ideology, medical and management knowledge, mobile internet, and other modern information technology are applied to actively carry out the "Internet plus" traditional Chinese medicine health management service model, and provide personalized, professional and intelligent traditional Chinese medicine health management services. By utilizing the Hangzhou Perimenopausal Women Healthcare Center, the Hangzhou Menopausal Healthcare Standardized and Specialized Outpatient Management Platform, the Hangzhou Maternal and Child Healthcare Specialized Union, and the Internet Hospital Women’s Menopausal Care Service Package, a three-tier prevention and control system for perimenopausal and postmenopausal osteoporosis is established based on the risk stratification of osteoporosis fractures, with the division of labor and cooperation of three-tier medical institutions as the main body, to achieve long-term effective management of chronic diseases throughout the entire course. |
