[摘要] 目的 探讨清胰导泄汤联合早期肠内营养治疗重症胰腺炎的效果。 方法 选取2020年5月至2022年8月在湖南中医药大学附属长沙市中医医院治疗的重症胰腺炎患者104例为研究对象,根据随机对照原则将其分为观察组(n=52)和对照组(n=52)。对照组患者采取早期肠内营养进行治疗,观察组患者采用清胰导泄汤联合早期肠内营养进行治疗。比较分析两组患者的临床疗效、胰腺炎酶类指标、炎症因子指标、急性生理与慢性健康评分(APACHE Ⅱ评分)和治疗安全性。 结果 治疗后,观察组患者在腹痛、腹胀缓解时间、胃肠道恢复时间和禁食时间更短,差异有统计学意义(P < 0.05)。患者血清中的血清淀粉酶(AMY)、脂肪酶(LIPA)、乳酸脱氢酶(LDH)、白细胞介素-7(IL-7)、白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)和单核细胞趋化因子-1(MCP-1)的含量均明显低于对照组(P < 0.05)。治疗后,观察组患者的APACHEⅡ评分明显低于对照组(P < 0.05)。 结论 清胰导泄汤联合早期肠内营养治疗重症胰腺炎的效果显著,能够降低患者的炎症因子水平,治疗安全性好。 |

[Abstract] Objective To explore the effect of Qingyi Daoxie Decoction combined with early enteral nutrition in the treatment of severe pancreatitis. Methods A total of 104 patients with severe pancreatitis who were treated at Changsha Hospital of Traditional Chinese Medicine from May 2020 to August 2022 were selected as the research subjects. They were divided into an observation group (n=52) and a control group (n=52) based on the principle of randomized control. The patients in the control group were treated with early enteral nutrition, while patients in the observation group were treated with Qingyi Daoxie Decoction combined with early enteral nutrition. The clinical efficacy, pancreatitis enzyme indicators, inflammatory factor indicators, acute physiology and chronic health evaluation scores (APACHEⅡ scores), and treatment safety of two groups of patients were compared and analyzed. Results After treatment, patients in the observation group showed better performance in terms of abdominal pain, abdominal distension relief time, gastrointestinal recovery time, and fasting time, with statistically significant differences (P < 0.05). Serum amylase (AMY), lipase (LIPA), lactate dehydrogenase (LDH), interleukin-7 (IL-7), interleukin-6 (IL-6), tumor necrosis factor (TNF-α) and the content of monocyte chemotactic protein-1 (MCP-1) in the patient’s serum of the observation group were significantly lower than that of the control group (P < 0.05). After treatment, the APACHE Ⅱ score of patients in the observation group was significantly lower than that of patients in the control group (P < 0.05). Conclusion The combination of Qingyi Daoxie Decoction and early enteral nutrition has a significant therapeutic effect on severe pancreatitis, which can reduce the level of inflammatory factors in patients and has good treatment safety. |
