| [摘要] 目的 观察加减五皮饮联合甘露醇治疗跟骨骨折术前肿胀的临床效果。方法 选取2020年5月至2022年5月湖南中医药大学第二附属医院骨伤科住院患者60例，自愿参与本临床疗效观察且符合诊断标准的60例跟骨骨折患者，通过随机数表法分为治疗组与对照组，每组各30例。对照组采用甘露醇治疗，治疗组在此基础上加用加减五皮饮内服。比较两组患者患肢肿胀程度、首次出现皮纹征时间、血液流变学指标、并发症发生率及药物不良反应。结果 对比治疗后第3、5、7天，治疗组肿胀程度均小于对照组，差异有统计学意义（P < 0.05）；治疗组首次出现皮纹征的时间为（4.97±0.89）d，明显短于对照组的（6.72±0.77）d，差异有统计学意义（P < 0.05）；治疗后两组全血黏度、血浆黏度、血沉各水平均较治疗前降低 （P < 0.05），治疗组全血黏度、血浆黏度及血沉指标均优于对照组，差异有统计学意义（P < 0.05）；治疗组术前并发症总发生率为16.67％，明显低于对照组的53.33％，差异有统计学意义（P < 0.05）；治疗期间，两组患者均未出现明显药物不良反应。结论 加减五皮饮联合甘露醇治疗跟骨骨折术前肿胀的效果确切。 |

| [Abstract] Objective To observe the clinical efficacy of modified Wupi Decoction combined with mannitol on preoperative swelling of calcaneal fracture. Methods A total of 60 inpatients with calcaneal fracture admitted to the Department of Orthopedics of the Second Hospital of Hunan University of Chinese Medicine from May 2020 to May 2022, who voluntarily participated in the clinical efficacy observation and met the diagnostic criteria were selected. And they were divided into the treatment group (n=30) and the control group (n=30) according to the random number table method. The control group was treated with mannitol, while the treatment group was combined with oral administration of modified Wupi decoction on this basis. The degree of swelling in patients’ affected limbs, the time of first appearance of dermatoglyphic sign, hemorheological indices, the incidence of complications and drug adverse reactions (ADRs) were compared between the two groups. Results On the 3rd, 5th and 7th day after treatment, the swelling degrees of the treatment group were all lower than those of the control group, with statistically significant differences (P < 0.05). The time of first appearance of dermatoglyphic sign in the treatment group was (4.97±0.89) d, remarkably shorter than (6.72±0.77) d in the control group, with statistically significant difference; After treatment, all levels of whole blood viscosity, plasma viscosity and erythrocyte sedimentation rate in the two groups were lower than those before treatment (P < 0.05), and the whole blood viscosity, plasma viscosity and erythrocyte sedimentation rate in the treatment group were all superior to those in the control group, with statistically significant differences (P < 0.05). The total incidence of preoperative complications in the treatment group was 16.67%, remarkably lower than 53.33% in the control group, with statistically significant difference (P < 0.05). During the treatment, there was no obvious drug ADRs in both groups of patients. Conclusion Modified Wupi decoction combined with mannitol is exactly effective in treating preoperative swelling of calcaneal fracture. |
