[摘要] 目的 探讨凝血指标和血尿素(UREA)水平与子痫前期(PE)发病的关联程度,旨在发现可以预测PE发生的化验指标。方法 选取2021年1—12月在河南省妇幼保健院分娩的220例孕妇为研究对象,PE患者60例,重度子痫前期(sPE)患者60例,正常孕妇100例,回顾性分析三组孕妇的临床资料、血凝指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D-二聚体(D-D)]和UREA水平,使用受试者工作特征曲线(ROC)分析血凝指标和UREA水平对PE发病的预测价值。 结果 三组年龄比较,差异无统计学意义(P > 0.05)。三组分娩孕龄、血压、体重指数(BMI)、新生儿出生体重、血凝指标(PT、APTT、D-D)、UREA比较,差异均有统计学意义(P < 0.05)。ROC曲线结果显示,APTT、D-D和UREA对PE有辅助诊断价值,曲线下面积(AUC)分别为0.72、0.80、0.84(P < 0.05)。 结论 血凝指标(PT、APTT、D-D)和UREA可反映PE早期凝血功能变化和肾功能损害,APTT、D-D和UREA对PE有预测价值。 |

[Abstract] Objective To investigate the correlation between coagulation indices, blood urea (UREA) levels and the onset of pre-eclampsia, in order to find laboratory indices that can predict the occurrence of pre-eclampsia. Methods A total of 220 pregnant women admitted to and giving birth in Maternal and Child Health Hospital of Henan Province from January to December 2021 were selected as the research objects, and they were divided into the group of pre-eclampsia (PE, n=60), the group of severe pre-eclampsia (sPE, n=60) and the group of normal pregnant women (n=100). The general clinical data, blood coagulation indices (prothrombin time [PT], activated partial thromboplastin time [APTT], D-dimer [D-D]) and UREA levels of pregnant women in the three groups were analyzed retrospectively. The predictive value of blood coagulation indices and blood urea levels on the onset of pre-eclampsia was analyzed by the receiver operating characteristic curve (ROC). Results There was no statistically significant difference in age in the three groups (P > 0.05). There were statistically significant differences in the gestational age at delivery, blood pressure, BMI, neonatal birth weight, blood coagulation indices (PT, APTT, D-D) and UREA in the three groups (P < 0.05). The ROC curve showed that APTT, D-D and UREA were of auxiliary diagnostic value for pre-eclampsia, and the area under the curve (AUC) were 0.72, 0.80 and 0.84, respectively (P < 0.05). Conclusion Blood coagulation indices (PT, APTT, D-D) and blood urea can reflect the changes of coagulation function and renal function damage in the early stage of pre-eclampsia, and APTT, D-D and blood urea have predictive value for pre-eclampsia. |
