[摘要] 持续存在的人乳头瘤病毒(HPV)感染是导致宫颈上皮内瘤变(CIN)和宫颈癌的重要因素。HPV在多因素调控下能被免疫系统自然清除,或导致宫颈病变进展,产生这两种结局的机制尚不明确。探究HPV感染向CIN方向发展的危险因素及致癌途径是诊治的关键。阴道微生物(VM)及相关代谢产物具有抗病毒、抗菌、抗肿瘤和调节免疫等作用。宫颈-阴道局部微生物组成改变和代谢紊乱会增加外源性感染和致癌的风险。细菌性阴道病(BV)和乳酸杆菌优势缺失是HPV感染危险因素。本文将综述VM及其代谢产物在HPV感染和宫颈病变中的影响和调控机制,为改善HPV感染转归、寻求疾病个体化治疗靶点和提供个性化诊疗策略提供理论依据。 |

[Abstract] Persistent human papillomavirus (HPV) infection is an important factor leading to cervical intraepithelial neoplasia (CIN) and cervical cancer. HPV can be naturally cleared by the immune system under the regulation of multiple factors, or may lead to the progression of cervical lesions, while the mechanism for these two outcomes is still unclear. Exploring the risk factors and carcinogenic pathways of HPV infection developing towards CIN is the key to the diagnosis and treatment. Vaginal microbiomes (VM) and related metabolites have the effects of anti-virus, anti-bacteria, anti-tumor and immune regulation. Changes in the microbial composition and metabolic disorders of the local cervical-vaginal area will increase the risk of exogenous infection and carcinogenesis. Bacterial vaginosis (BV) and lack of superiority of Lactobacillus are risk factors for HPV infection. This article will review the effects and regulatory mechanisms of VM and their metabolites in HPV infection and cervical lesions, in order to provide a theoretical basis for improving the outcome of HPV infection, seeking individual treatment targets of the disease and providing individual diagnosis and treatment strategies. |
