[摘要] 目的 探讨经胸超声心动图(TTE)联合下肢静脉加压超声(CUS)及D-二聚体对疑似急性肺动脉栓塞(APE)的诊断价值。 方法 收集山西省人民医院2020年5月至2021年5月可疑APE且影像资料及住院病历完善的患者共计176例,其中APE组51例,非APE组125例。TTE主要观察指标包括右心室扩大、右心室游离壁运动减低、三尖瓣反流速度加快、三尖瓣环收缩期移位减低,CUS主要观察下肢静脉有无血栓。记录患者D-二聚体检查结果。任何一项参数阳性认为超声诊断APE阳性。以CT肺动脉造影作为诊断标准。 结果 TTE联合CUS诊断APE的敏感度(84.31%)高于TTE或CUS单独使用,准确率(73.86%)略低于CUS单独使用,但AUC(0.770)最大,诊断效能高于单独使用TTE、CUS及TTE联合CUS及D-二聚体。 结论 TTE联合CUS对于诊断APE是一种行之有效的检查方法。 |

[Abstract] Objective To investigate the diagnostic value of transthoracic echocardiography (TTE) combined with lower extremity veins compression ultrasound (CUS) and D-dimer in suspected acute pulmonary embolism (APE). Methods A total of 176 patients with suspected APE and completed imaging data and medical records from May 2020 to May 2021 in Shanxi Provincial People’s Hospital were collected. And they were divided into the APE group (n=51) and the non-APE group (n=125). The main outcome measures of TTE included enlargement of right ventricle, reduction of free wall motion of right ventricue, acceleration of tricuspid regurgitation, and reduction of systolic displacement of tricuspid annulus. CUS was mainly used to observe whether there was thrombus in lower extremity veins. D-dimer examination results of patients were recorded. If any parameter was positive, the ultrasound diagnosis of APE was considered as positive. CTPA was used as the diagnostic standard. Results The sensitivity (84.31%) of TTE combined with CUS in the diagnosis of APE was higher than that of TTE or CUS alone, and the accuracy (73.86%) was slightly lower than that of CUS alone, but the AUC (0.770) was the highest. The diagnostic efficiency was higher than that of TTE, CUS and TTE combined with CUS and D-dimer alone. Conclusion TTE combined with CUS is an effective examination method to diagnose APE. |
