[摘要] 目的 探究慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的有效治疗方案,并评估无创呼吸机的应用效果。 方法 选取邳州市中医院2020年1月至2021年12月收治的COPD合并Ⅱ型呼吸衰竭患者76例,采用随机数表法分为两组。对照组38例应用常规治疗,研究组38例在对照组基础上实施无创呼吸机治疗,比较两组的临床效果。 结果 治疗前,两组的血氧分压(PaO2)、pH值、二氧化碳分压(PaCO2)比较,差异无统计学意义(P > 0.05);治疗后,研究组PaO2、pH值高于对照组,研究组PaCO2低于对照组,差异有统计学意义(P < 0.05);治疗前,两组第1秒用力呼气容积(FEV1)、最大呼气容积(FVC)、FEV1/FVC比较,差异无统计学意义(P > 0.05);治疗后,研究组FEV1、FVC、FEV1/FVC高于对照组,差异有统计学意义(P < 0.05);研究组治疗总有效率高于对照组,差异有统计学意义(P < 0.05)。 结论 相较于常规治疗方案,COPD合并Ⅱ型呼吸衰竭应用无创呼吸机的实施效果更为显著,具有广泛的借鉴意义,值得临床推广应用。 |

[Abstract] Objective To explore an effective treatment plan for chronic obstructive pulmonary disease (COPD) with typeⅡrespiratory failure, and focus on the evaluation of the application effect of non-invasive ventilator. Methods A total of 76 cases of COPD patients with type Ⅱ respiratory failure admitted to Pizhou Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected, and were divided into two groups for analysis by random number table method. The control group (n=38) were treated with conventional therapy, and the study group (n=38) were treated with non-invasive ventilator on the basis of the control group. The clinical effects of the two groups were compared. Results Before treatment, There were no significant diffences of the blood oxygen partial pressure (PaO2), pH value and carbon dioxide partial pressure (PaCO2) between the two groups (P > 0.05); After treatment, PaO2 and pH in the study group were higher than those in the control group, while PaCO2 in the study group was lower than those in the control group, with statistically significant difference (P < 0.05); Before treatment, there was no significant difference between the two groups in the first second forced expiratory volume (FEV1), maximum expiratory volume (FVC) and FEV1/FVC) (P > 0.05); After treatment, FEV1, FVC and FEV1/FVC in the study group were significantly higher than those in the control group (P < 0.05); The total effective rate of the study group was higher than that of the control group, with statistically significant difference (P < 0.05). Conclusion Compared with the conventional treatment scheme, the implementation effect of noninvasive ventilator for COPD with typeⅡrespiratory failure is more significant, which has a broad reference significance and is worthy of clinical promotion. |
