      [摘要] 目的 探究肺结核患者血清促甲状腺激素(TSH)和糖化血红蛋白(HbA1c)的关系,旨在为临床治疗肺结核患者提供科学依据。 方法 选取2019年1月至2020年6月牡丹江市康安医院发热门诊及结核疗区82例肺结核患者为研究对象,检测患者的TSH水平和HbA1c水平,以血清TSH为2.5 mU/L为界将患者分为两组,其中TSH>2.5 mU/L组患者42例,TSH≤2.5 mU/L组患者40例;以4%为界,将患者HbA1c分为两组,HbA1c≤4%组49例、4%2.5 mU/L组FPG、HbA1c、TG、LDL-C与TSH≤2.5 mU/L组比较,差异有统计学意义(P < 0.05);HbA1c≤4%组、4<%HbA1c≤7%组FT4比较,差异无统计学意义(P > 0.05),FT3、TSH比较,差异有统计学意义(P < 0.05)。Pearson法分析结果显示,TSH与TC、HDL-C无相关性(P > 0.05),与FPG、HbA1c、TG、LDL-C呈正相关(P < 0.05),HbA1c与FT3呈正相关(P < 0.05),与FT4无相关性(P > 0.05)。 结论 肺结核患者血清TSH和HbA1c呈正相关,TSH升高患者有糖脂代谢风险,HbA1c升高患者有甲状腺功能障碍风险。

      [Abstract] Objective To explore the relationship between thyroid stimulating hormone (TSH) and glycated hemoglobin A1c (HbA1c) in patients with pulmonary tuberculosis, aiming to provide a scientific basis for the clinical treatment of patients with pulmonary tuberculosis. Methods A total of 82 patients with pulmonary tuberculosis in the Fever Clinic and Tuberculosis Treatment Ares of Mudanjiang Kang'an Hospital from January 2019 to June 2020 were selected as the study subjects, and were detected for TSH levels and HbA1c levels. These patients were divided into two groups with a serum TSH of 2.5 mU/L as the boundary, with 42 patients in the TSH > 2.5 mU/L group and 40 patients in the TSH ≤ 2.5 mU/L group. And patients were also divided into another two groups with HbA1c value of 4% as the boundary, with 49 patients in the HbA1c ≤ 4% group and 33 patients in the 4% < HbA1c ≤ 7% group. The glycolipid metabolic indexes and thyroid hormone levels of patients with different TSH levels and HbA1c levels were compared, and the correlation between TSH and HbA1c was analyzed using the Pearson method. Results There were statistically significant differences between the TSH > 2.5 mU/L group and the TSH ≤ 2.5 mU/L group in fasting plasma glucose (FPG), HbA1c, triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) (P < 0.05). There was no statistically significant difference between the HbA1c ≤ 4% group and the 4% < HbA1c ≤ 7% group in free thyroxine (FT) 4 (P > 0.05), while there were statistically significant differences between the two groups in FT3 and TSH (P < 0.05). Pearson analysis showed that TSH was not correlated with TC and HDL-C (P > 0.05), but positively correlated with FPG, HbA1c, TG and LDL-C (P < 0.05), and HbA1c was positively correlated with FT3 (P < 0.05), but not correlated with FT4 (P > 0.05). Conclusion TSH is positively correlated with HbA1c in patients with pulmonary tuberculosis,suggesting that patients with elevated TSH may at a risk of glycolipid metabolism, and patients with elevated HbA1c may at a risk of thyroid dysfunction.
