| [摘要] 眼肌型重症肌无力是重症肌无力的常见类型。近年来，中西医结合治疗本病取得了良好成效。眼肌型重症肌无力证属“本虚标实”，以“脾肾虚损”为本，“湿热”为标。脾肾亏虚，脾之运化、肾之气化失司，水液停聚，水湿内生；脾胃功能紊乱，湿停中焦，郁而化热，或久病耗伤肾之阴液，阴虚火旺，虚火上炎，而成湿热。脾肾为湿热所困则会加重脏腑虚损，使肌肉筋脉失养，骨骼精髓不充，而致四肢痿软，眼睑肌肉松弛。谢道俊教授结合前人与自身临证经验，提出眼肌型重症肌无力治宜在“治痿独取阳明”、补益脾肾基础上，加以清热化湿。同时运用丹皮治疗本病颇有良效。本文归纳总结谢道俊教授治疗眼肌型重症肌无力经验，并附临证医案一则。 |

| [Abstract] Ocular myasthenia gravis is a common type of myasthenia gravis. In recent years, the combination of Chinese medicine and Western medicine has achieved good efficacy in treating this disease. Ocular myasthenia gravis is characterized by "deficiency in origin and excess in superficiality" with "spleen and kidney deficiency" as the origin and "dampness and heat" as the superficiality. Deficiency of spleen and kidney can result in diminished transportation and transformative role of the spleen and reduced gasification function of the kidney, further the stagnation of water and fluid, and the endogenesis of water and dampness. The spleen and stomach function disorder, the retention of damp-heat in the middle-Jiao, the stagnation of Qi transforming into heat, or the loss of Yin-fluid due to the chronic disease involving kidney can lead to yin deficiency and fire effulgence, and deficiency-fire flaring upward, thus resulting in the formation of damp-heat. When the spleen and kidney are plagued by damp-heat, it will aggravate the deficiency and loss of viscera, and make the muscles and veins lose their nourishment, and the bone essence not sufficient, resulting in flaccid limbs and floppy eyelid muscles. Based on the previous experience and his own clinical experience, Professor Xie Daojun proposed that the treatment of ocular myasthenia gravis should take "treatment for flaccidity aims at yangming meridian" as the core and adopt the strategy of tonifying the spleen and kidney, supplemented with clearing away heat and dampness. Meanwhile, the treatment of this disease with Tree Peony Bark (Dan Pi) is quite effective. This paper summarizes Professor Xie Daojun’s experience in treating ocular myasthenia gravis, and a clinical case is attached. |
