      [摘要] 目的 探讨乳甲外科住院医师规范化培训(简称“住培”)教学实践中形成性评价的教学效果。 方法 选取2019年9月至2021年8月在青海大学附属医院乳甲外科轮转的住院医师104人,随机分为形成性评价组(n=47)和终结性评价组(n=57),形成性评价组采用迷你临床演练评估(Mini-CEX)、操作技能直接观察评估考核(DOPS)及自我测评和问卷调查等方法,终结性评价组采用传统评价方法,分析教学效果和住院医师对形成性评价的满意度。 结果 形成性评价组1、2、3个月末的Mini-CEX和DOPS考核成绩比较,差异有统计学意义(P=0.000);住院医师学习和教师带教效果的自我测评末次均高于首次(P=0.000)。形成性评价组出科临床实践能力考核成绩高于终结性评价组(P=0.000)。形成性评价组中91.49%认为有助于理论和实践技能提高,97.87%认为有助于诊疗患者综合能力提高,93.62%认为有助于团队沟通协作能力,87.23%认为有助于激发学习积极性,93.62%对形成性评价感到满意。 结论 采用Mini-CEX联合DOPS的形成性评价方法用于外科住培教学实践,可提高住院医师临床实践能力,对医学人才培养具有应用价值。

      [Abstract] Objective To explore the teaching effect of formative evaluation in the teaching practice of breast and thyroid surgery residency training. Methods A total of 104 residents who were rotated in the Department of Breast and Thyroid Surgery of Affiliated Hospital of Qinghai University from September 2019 to August 2021 were randomly divided into formative evaluation group (n=47) and summative evaluation group (n=57). The formative evaluation group used formative evaluation methods such as mini clinical evaluation exercise(Mini-CEX), direct observation of procedural skills(DOPS) and self assessment, residents questionnaire survey, and the summative evaluation used traditional summative evaluation method. Analyze the teaching effect and residents’ satisfaction with formative evaluation. Results At the end of 1st, 2nd and 3rd month, the Mini-CEX and DOPS tests scores were statistically significant (P=0.000). The self-assessment of residents’ learning effect and teachers’ teaching effect was higher in the last assessment than in the first assessment (P=0.000).The results of clinical practice ability assessment in the formative evaluation group were higher than those in the summative evaluation group (P=0.000). Questionnaire results of the formative evaluation group: 91.49% of residents thought it was helpful to improve the level of theoretical knowledge and practical skills, 97.87% thought it was helpful to improve the comprehensive ability of diagnosis and treatment of patients, 93.62% thought it was helpful to team communication and collaboration, 87.23% thought it was helpful to motivate the learing, 93.62% of residents were satisfied with the formative assessment. Conclusion The use of the formative evaluation method of Mini-CEX combined with DOPS can significantly improve the clinical practice ability of surgical residents, and has application value for the training of medical talents.
