      [摘要] 目的 本研究通过比较乳腺非特殊型浸润性癌外泌体miroRNA表达谱差异,寻找可用于诊断的外泌体miroRNA标志物,评估外泌体miroRNA作为乳腺非特殊型浸润性癌诊断标志物的潜在应用价值。 方法 选取2018年3—5月深圳市人民医院乳腺非特殊型浸润性癌5例早期患者和5例晚期患者为研究对象,另取健康体检者2例为对照组,比较分析血浆中外泌体miroRNA表达谱。 结果 研究发现与对照组比较,在乳腺非特殊型浸润性癌组中有14个外泌体miroRNA的表达量差异有显著统计学意义(P < 0.01)。这些差异表达的外泌体miroRNA靶基因的富集通路的重合性也达到60%~95%,说明大部分差异表达的外泌体miroRNA同时在乳腺非特殊型浸润性癌的发生和转移中发挥作用。预后分析发现与对照组比较,4个外泌体miroRNA(miRNA-451a、miRNA-493-5p、miRNA-409-3p和miRNA-151a-5p)预后情况与研究预期一致,有潜力同时适用于早期和晚期乳腺非特殊浸润性癌的血液学诊断。 结论 本研究为外泌体miroRNA检测成为一种无创的乳腺非特殊型浸润性癌血液学检测方法提供理论参考。

      [Abstract] Objective To search the miroRNA markers in exosomes that can be used for diagnosis by comparing the differences of miroRNA expression profile in exosomes of non-specific infiltrative breast cancer, and to evaluate the potential application value of exosomal microRNA as a diagnostic marker of non-specific infiltrative breast cancer. Methods A total of 5 early-stage patients and 5 late-stage patients with non-specific infiltrative breast cancer in Shenzhen People’s Hospital from March to May 2018 were selected as the research objects and taken as the non-specific infiltrative breast cancer group, and 2 healthy examined people were taken as the control group. The miroRNA expression profiles of plasma exosomes were compared and analyzed. Results It was found that compared with the control group, there were statistically significant differences in the expression levels of 14 exosomal microRNA in the non-specific infiltrative breast cancer group (P < 0.01). The coincidence of enrichment pathways of these differentially expressed exosomal microRNA target genes also reached 60%-95%, indicating that most differentially expressed exosomal microRNA played a role in the occurrence and metastasis of non-specific infiltrative breast cancer at the same time. Prognosis analysis found that compared with the control group, the prognosis of 4 exosomal microRNA (miRNA-451a, miRNA-493-5p, miRNA-409-3p and miRNA-151a-5p) was consistent with the research expectation, and it has the potential to be applied to the hematological diagnosis of early-stage and late-stage non-specific infiltrative breast cancer. Conclusion This study provides a theoretical reference for the detection of exosomal microRNA as a noninvasive hematological method for non-specific invasive breast cancer.
