      [摘要] 目的 探讨生物反馈电刺激联合综合运动治疗产后盆底功能障碍的效果。 方法 回顾性分析厦门大学附属妇女儿童医院2021年6—12月收治的95例产后盆底功能障碍患者,按照治疗方式不同分为对照组和观察组,对照组47例予以综合运动疗法治疗,观察组48例在对照组基础上予以生物反馈电刺激治疗。观察两组患者盆底肌肌力、尿失禁、盆腔脏器脱垂情况以及盆底组织状态。 结果 干预2个月后,观察组肌力分级高于对照组,差异有统计学意义(P < 0.05);观察组尿失禁程度轻于对照组,差异有统计学意义(P < 0.05);观察组盆腔脏器脱垂情况优于对照组,差异有统计学意义(P < 0.05);观察组膀胱颈旋转角(θ)、压力状态下尿道膀胱后角(AS)和静息状态下尿道膀胱后角(Ar)角度小于对照组,差异有统计学意义(P < 0.05)。 结论 对产后盆底功能障碍患者予以生物反馈电刺激联合综合运动干预,可提高盆底肌肌力和盆底组织状况,改善尿失禁以及盆腔脏器脱垂情况。

      [Abstract] Objective To explore the effect of biofeedback electrical stimulation combined with comprehensive exercise on postpartum pelvic floor dysfunction. Methods A total of 95 patients with postpartum pelvic floor dysfunction admitted to Women’s and Children’s Hospital Affiliated to Xiamen University from June to December 2021 were retrospectively analyzed. According to the different treatment methods, they were divided into the control group and the observation group. 47 patients in the control group were treated with comprehensive exercise therapy, while 48 patients in the observation group were treated with biofeedback electrical stimulation based on the treatment of the control group. The pelvic floor muscle strength, urinary incontinence, pelvic organ prolapses and pelvic floor tissue status of the two groups were observed. Results After 2 months of intervention, the grade of muscle strength of the observation group was higher than that of the control group, with statistically significant difference (P < 0.05). The degree of urinary incontinence in the observation group was lighter than that in the control group, with statistically significant difference (P < 0.05). The pelvic organ prolapses in observation group was better than that in control group, with statistically significant difference (P < 0.05). The rotation angle of the bladder neck (θ), the posterior urethrovesical angle during straining (AS) and the posterior urethrovesical angle at rest (Ar) in the observation group were smaller than those in the control group, with statistically significant differences (P < 0.05). Conclusion Biofeedback electrical stimulation combined with comprehensive exercise intervention for patients with postpartum pelvic floor dysfunction can enhance pelvic floor muscle strength and pelvic floor tissue status, as well as improve urinary incontinence and pelvic organ prolapses.
