      [摘要] 目的 探讨水激光联合盐酸米诺环素软膏治疗种植体周围炎的早期预后及种植体软硬组织治疗效果。 方法 选取西宁市口腔医院种植科2018年11月至2020年12月就诊的单颗种植体周围炎患者36例,按照随机数表法将其分为研究组(水激光治疗组)和对照组(传统刮治组),每组各18例,即每组18枚种植体。对照组应用传统种植体周围刮治术进行治疗,研究组应用水冷激光行种植体周围清理术,术后创面联合应用盐酸米诺环素软膏。两组均随访3个月。根据牙龈指数、探诊出血指数、X-线片中骨水平变化指标判断治疗是否有效。 结果 研究组总体治疗有效率高于对照组,差异有统计学意义(P < 0.05)。治疗后两周复查时,两组种植体周围探诊出血指数和牙龈指数均低于治疗前,差异有统计学意义(P < 0.05);研究组牙龈指数较对照组低(P < 0.05);两组间种植体周围探诊出血指数,差异无统计学意义(P > 0.05)。治疗后3个月复查时,两组的种植体周围探诊深度均较治疗前减小,差异有统计学意义(P < 0.05);研究组种植体周围探诊深度显著低于对照组(P < 0.05)。 结论 水冷激光联合盐酸米诺环素软膏治疗种植体周围炎可以获得较好的临床效果,早期预后较好。

      [Abstract] Objective To investigate the early prognosis of water laser combined with minocycline hydrochloride ointment in the treatment of peri-implantitis and the effects of implant on soft and hard tissue treatment. Methods A total of 36 patients with peri-implantitis involving single implant visiting the Department of Implantology of Xining Stomatological Hospital from November 2018 to December 2020 were selected and randomly divided into the study group (water laser treatment group) and the control group (traditional curettage group) according to the random number table method. There were 18 patients in each group, that is, 18 implants in each group. The control group was treated with traditional peri-implant curettage, while the study group was treated with water cooled laser for peri-implant cleaning and the postoperative wound was treated with minocycline hydrochloride ointment. Both groups were followed up for 3 months. The observation indicator included the efficacy of the treatment determined based on the gingival index, bleeding on probing, and bone level change indicators on the X-ray film. Results The overall effective rate of the study group was higher than that of the control group, with statistically significant difference (P < 0.05). At the review two weeks after treatment, the peri-implant bleeding on probing and gingival index of the two groups were lower than those before treatment, and the differences were statistically significant (P < 0.05). The gingival index of the study group was lower than that of the control group (P < 0.05). There was no statistically significant difference in peri-implant bleeding on probing between the two groups (P > 0.05). At the review three months after treatment, the peri-implant probing depth of the two groups decreased compared with that before treatment, and the difference was statistically significant (P < 0.05). The peri-implant probing depth in the study group was significantly less than that in the control group (P < 0.05). Conclusion Water cooled laser combined with minocycline hydrochloride ointment can achieve good clinical effect in the treatment of peri-implantitis, and the early prognosis is good.
