| [摘要] 目的 探讨圆锥型套筒冠义齿修复对牙周病伴牙列缺损的治疗效果及对咀嚼效能、菌斑指数和牙齿松动度的影响。方法 选取2015年6月至2017年6月至西安临潼康复疗养中心口腔修复科就诊的重度牙周病伴发牙列缺损患者40例，患者均接受基础治疗，并行圆锥型套筒冠义齿修复。检测患者治疗后自我感觉，治疗前后菌斑指数、出血指数、探诊深度、附着丧失和牙龈指数，治疗后1年基牙松动度和治疗后5年基牙松动度，并进行统计和分析。结果 治疗的总有效率为95.0%；患者对固位力满意率为75.0%，患者对咀嚼功能满意率为80.0%，患者对美观度满意率为72.5%；治疗后，患者菌斑指数、出血指数、探诊深度、附着丧失和牙龈指数均显著下降（t=49.621，P＜0.01；t=78.375，P＜0.01；t=68.783，P＜0.01；t=42.555，P＜0.01；t=2.297，P＜0.05）；治疗后1年，患者基牙松动程度显著改善（Z=213.115，P＜0.01）；与治疗后1年比较，治疗后5年患者基牙松动程度改善（Z=42.020，P＜0.01）。结论 圆锥型套筒冠义齿修复对牙周病伴牙列缺损的患者具有良好的短期和长期治疗效果，能显著改善患者的咀嚼效能、菌斑指数和牙齿松动度。 |

| [Abstract] Objective To investigate the therapeutic efficacy of conical telescopic denture on periodontal disease complicated with dentition defect and its impacts on masticatory efficiency, plaque index and tooth mobility. Methods A total of 40 patients with severe periodontal disease complicated with dentition defect in Center for Rehabilitation and Recuperation in Xi'an Lintong from June 2015 to June 2017 were selected. All patients were given basic treatment, followed by conical telescopic denture. The patients' self-feeling after treatment, the patients' plaque index, hemorrhage index, probing index, attachment loss and gingival index before and after treatment, and abutment mobility 1 year after treatment and abutment mobility 5 years after treatment were tested, counted and analyzed. Results The total efficacy rate of the treatment was 95.0%. The satisfaction rate of patients with retention was 75.0%, the satisfaction rate of patients with masticatory function was 80.0%, and the satisfaction rate of patients with aesthetics was 72.5%. After treatment, plaque index, hemorrhage index, probing index, attachment loss and gingival index of patients were decreased significantly (t=49.621, P＜0.01; t=78.375, P＜0.01; t=68.783, P＜0.01; t=42.555, P＜0.01; t=2.297, P＜0.05). One year after treatment, the abutment mobility was improved significantly (Z=213.115, P＜0.01). Compared with 1 year after treatment, abutment mobility was improved 5 years after treatment (Z=42.020, P ＜ 0.01). Conclusion Conical telescopic denture restoration has good short-term and long-term therapeutic efficacies on patients with periodontal disease and dentition defect, and can significantly improve the masticatory efficiency, plaque index and tooth mobility. |
