      [摘要] 目的 观察复合营养干预方案(维生素D、维生素A和锌)对于南宁市牛奶蛋白过敏症(CMPA)患儿的营养和生长发育的影响。 方法 统计分析 2019 年 3 月1日至 2019 年 12 月1日在南宁市第八人民医院及宾阳县人民医院、马山县人民医院、南宁市武鸣区妇幼保健医院、宾阳县妇幼保健院、上林县人民医院、上林县妇幼保健院、横县人民医院儿科门诊及住院部就诊的1~3岁CMPA患儿349例;将其分为研究组与对照组,研究组182例CMPA患儿采用氨基酸奶粉联合复合维生素D、A和锌的综合营养方案,对照组167例CMPA患儿仅采用氨基酸奶粉干预。测定两组患儿身高、体重、体重指数(BMI)和血清维生素D、维生素A和锌,以及血清白蛋白、前白蛋白、转铁蛋白和视黄醇结合蛋白浓度,随访6个月后进行第二次测量,对比分析两组各指标差异。 结果 干预前研究组和对照组所有测量指标均无统计学差异,干预6个月后,研究组在身高增长、体重、 体重增长、BMI值均高于对照组(P<0.05)。研究组的血清维生素A,维生素D和锌水平显著高于对照组,研究组的血清铁蛋白、视黄醇结合蛋白水平显著高于对照组(P<0.05)。 结论 本研究所提出的综合营养补充方案比单一氨基酸奶粉补充方案对于南宁市1~3岁CMPA患儿的生长发育及提高维生素水平效果更好,值得临床推广使用。

      [Abstract] Objective To observe the impacts of compound nutrition intervention scheme (vitamin D, vitamin A and zinc) on nutrition and growth and development of children with cow milk protein allergy (CMPA) in Nanning. Methods A total of 349 cases of children with CMPA aged 1-3 years admitted to various of outpatient and inpatient departments of pediatrics of the Eighth People's Hospital of Nanning,Binyang County People's Hospital, Mashan County People's Hospital,Nanning Wuming District Maternal and Child Healthcare Hospital,Binyang Maternal and Child Health Hospital, Shanglin People's Hospital, Shanglin Maternal and Child Health Hospital, Hengxian People's Hospital from March 1, 2019 to December 1, 2019 were statistically analyzed, and they were divided into the study group (n=182) and the control group (n=167). Children with CMPA in the study group were treated with amino acid milk powder combined with vitamin D, vitamin A and zinc, while those in the control group were singly treated with amino acid milk powder. Height, weight, BMI index, serum vitamin D, vitamin A and zinc, serum albumin, prealbumin, transferrin and retinol binding protein were determined in the two groups of children, and the second determination was made after 6 months of follow-up. Meanwhile, the differences between the two groups were compared and analyzed. Results Before intervention, there was no significant difference in all determination indexes between the study group and the control group. After 6 months of intervention, the height growth, weight gain and BMI value of the study group were higher than those of the control group (P<0.05).
