      [摘要] 目的 探究重症医学科呼吸衰竭患者应用同步间歇指令通气与适应性支持通气模式机械通气的临床效果。 方法 本研究均选自 2017 年 2 月~ 2019 年 2 月在本院重症医学科进行治疗的 89 例呼吸衰竭患者,按照入院治疗先后时间分为实验组(n=45)和对照组(n=44),对照组开展同步间歇指令通气联合压力支持通气,实验组开展适应性支持通气,分析比较两种通气方式的临床效果。 结果 实验组和对照组在平均气道压、气道平台压、气道峰压、通气时间、潮气量、指令呼吸频率、重症医学科住院时间、困难撤机率以及 PCT、WBC、CRP 炎性指标上比较,差异有统计学意义(P < 0.05)。 结论 重症医学科呼吸衰竭患者应用适应性支持通气模式机械通气的临床效果较好,能够有效改善患者呼吸力学,减少通气时间和炎性反应。

      [Abstract] Objective To explore the clinical curative effects of synchronous intermittent command ventilation and adaptive support ventilation mechanical ventilation in treatment of patients with respiratory failure in department of critical care medicine. Methods 89 patients with respiratory failure who were treated in department of critical care medicine in our hospital from February 2017 to February 2019 were selected as research objects in the study.According to the order of admission,they were divided into the experimental group (n=45) and the control group (n=44).The control group was given synchronous intermittent command ventilation combined with pressure support ventilation while the experimental group was given adaptive support mechanical ventilation.Clinical curative effects of two ventilation methods were analyzed and compared. Results There were statistically significant differences in mean airway pressure, airway platform pressure,airway peak pressure,ventilation time,tidal volume,command respiratory rate,length of stay in ICU,rate of difficult withdrawal and inflammatory indexes such as PCT,WBC and CRP between the experimental group and the control group (P < 0.05). Conclusion Adaptive support ventilation mechanical ventilation in treatment of patients with respiratory failure in department of critical care medicine has better clinical curative effects,which can effectively improve respiratory mechanics and reduce ventilation time and inflammatory response.
