      [摘要] 目的 了解本地区正常孕妇和新生儿维生素 A 缺乏状况,研究孕母维生素 A 水平对新生儿脐带血维生素 A 水平、分娩孕周及出生体重的影响。 方法 选取我院 2017 年 6 月~ 2018 年 6 月分娩的正常孕妇 597 名,检测静脉血及其新生儿脐带血维生素 A 水平,同时记录分娩孕周及出生体重等数据,随后采用 SPSS 软件进行统计学分析。 结果 本地区孕母及新生儿维生素 A 缺乏的比例分别为 23.45% 和 93.80%,高龄产妇组分娩孕周早于非高龄产妇组 [(38.93±1.05)周 vs (39.28±1.20)周,P=0.010];维生素 A 正常组孕妇脐带血维生素 A 浓度高于维生素 A 缺乏组 [(0.21±0.06)mg/L vs (0.19±0.04)mg/L,P=0.003],但出生体重则低于维生素 A 缺乏组 [(3.18±0.39)kg vs (3.29±0.45)kg,P=0.014];孕母维生素 A 缺乏是新生儿维生素 A缺乏的危险因素(OR=3.457,P=0.043)。 结论 本地区孕妇维生素 A 缺乏情况仍然比较严重严重,新生儿脐带血维生素 A 水平普遍较低,与孕母维生素 A 水平呈正相关性,并可影响新生儿出生体重,有必要制订适宜的孕期维生素 A 补充方案,保障母婴健康。

      [Abstract] Objective To understand the status of vitamin A deficiency in normal pregnant women and newborns in this area and to study the effects of maternal vitamin A level on vitamin A level of neonatal cord blood, gestational age and birth weight. Methods 597 normal pregnant women who delivered in our hospital from June 2017 to June 2018 were selected and their venous blood and neonatal cord blood vitamin A levels were detected.Data such as gestational age and birth weight were also recorded.Then SPSS software was used to statistically analyze. Results The proportion of pregnant mothers and newborns with vitamin A deficiency in the region was 23.45% and 93.80% respectively.The gestational age of delivery in the elderly group were earlier than those in the non-elderly group [(38.93±1.05) weeks vs (39.28±1.20) weeks,P=0.010].The concentration of vitamin A in umbilical cord blood of normal pregnant women was higher than that of vitamin A deficiency group [(0.21±0.06)mg/L vs (0.19±0.04)mg/L,P=0.003].However,the birth weight was lower than that of the vitamin A deficiency group [(3.18±0.39)kg vs (3.29±0.45)kg,P=0.014]. Maternal vitamin A deficiency was a risk factor for neonatal vitamin A deficiency (OR=3.457,P=0.043). Conclusion The vitamin A deficiency in pregnant women in this area is still serious.The vitamin A level in cord blood of newborns is generally low,which is positively correlated with maternal vitamin A level and can affect the birth weight of newborns.It is necessary to formulate a suitable vitamin A supplement program during pregnancy to ensure maternal and child health.
