      [摘要] 目的 探讨合理选择穿刺部位对降低新生儿 PICC 置管后导管相关血流感染(CLABSI)率的影响。方法 选取 2012 年 9 月~ 2017 年 9 月本院 PICC 置管后新生儿 1167 例,依据静脉穿刺部位分为大隐组(n=205例)、腋组(n=105 例)、股组(n=67 例)、贵要组(n=372 例)、正中组(n=214 例)、头组(n=204 例),比较六组CLABSI、留置时间、非计划拔管。结果 在 CLABSI 率、非计划拔管率方面,大隐组、贵要组、腋组明显低于正中组、头组,正中组、头组明显低于股组,差异有统计学意义(P < 0.05);在留置时间方面,大隐组、贵要组、腋组明显高于正中组、头组,正中组、头组明显高于股组,差异有统计学意义(P < 0.05)。结论 合理选择穿刺部位可有效降低新生儿 PICC 置管后 CLABSI 率,其中大隐、贵要、腋静脉可作为首选穿刺部位,并可有效减少非计划拔管而延长留置时间,值得临床作进一步推广。

      [Abstract] Objective To explore the effects of reasonable selection of puncture site on reducing the catheterassociated bloodstream infection (CLABSI) rate after PICC catheterization in newborns. Methods 1167 newborns who underwent PICC catheterization in our hospital from September 2012 to September 2017 were selected. According to the location of venipuncture, they were divided into the great saphenous group (n=205), the axillary group (n=105), the femoral group (n=67), the basilic group (n=372), the median group (n=214), and the head group (n=204). CLABSI, indwelling time and unplanned extubation of six groups were compared. Results CLABSI rate and unplanned extubation rate of the great saphenous group, the basilic group and the axillary group were significantly lower than those of the median group and the head group. CLABSI rate and unplanned extubation rate of the median group and the head group were significantly lower than those of femoral group. The differences were statistically significant(P< 0.05). Indwelling time of the great saphenous group, the basilic group and the axillary group were significantly higher than those of the median group and the head group. Indwelling time of the median group and the head group were significantly higher than those of femoral group. The differences were statistically significant(P < 0.05). Conclusion The reasonable selection of puncture site can effectively reduce the CLABSI rate after PICC catheterization in newborns. Great saphenous, basilic and axillary veins can be used as the preferred puncture site, which can effectively reduce the unplanned extubation and prolong the indwelling time. It’s worthy of further clinical promotion.
