      [摘要] 目的 探究阿奇霉素联合不同途径糖皮质激素对慢性阻塞性肺疾病急性加重(AECOPD)患者肺功能改善效果。方法 选取我院于 2016 年 9 月~ 2018 年 6 月收治的 90 例 AECOPD 患者为研究对象,随机分为三组,对照组采用常规治疗的基础上加用阿奇霉素,联合雾化组在常规治疗基础上给予阿奇霉素联合雾化吸入糖皮质激素(布地奈德混悬液),联合全身组患者在常规治疗的基础上给予阿奇霉素联合全身使用糖皮质激素(甲波尼龙),比较三组患者治疗前后的肺功能改善效果以及不良反应发生情况。结果 三组患者治疗一周后 FEV1、FEV1/ FVC 及 PEF 值均高于治疗前(P < 0.05),但联合雾化组和联合全身组患者治疗前后 FEV1、FEV1/ FVC 及 PEF 差值明显高于对照组,差异具有统计学意义(P < 0.05),三组患者治疗后 PaO2 明显升高,PaCO2 水平较治疗前明显降低(P < 0.05),治疗前后 PaO2 差值、PaCO2 差值联合雾化组以及联合全身组明显高于对照组(P < 0.05),联合全身组呕吐、低钾血症、血糖异常、腹部不适人数明显多于对照组以及联合雾化组,差异有统计学意义(P < 0.05)。结论 阿奇霉素联合糖皮质激素可有效改善 AECOPD 患者肺功能,雾化吸入糖皮质激素与全身使用糖皮质激素改善 AECOPD 患者肺功能疗效一致,但雾化吸入可降低不良反应的发生率。

      [Abstract] Objective To investigate the effect of azithromycin combined with different pathways of glucocorticoids on pulmonary function improvement in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 90 patients undergoing AECOPD were assigned into three groups according to the table of random number. The control group was treated with azithromycin on the basis of conventional treatment. The combined nebulization group was given azithromycin combined with nebulized inhaled corticosteroid (Budesonide suspension) on the basis of conventional treatment. The patients in the systemic group were given azithromycin on the basis of conventional treatment combined with systemic glucocorticoids (Methylprednisolone). The improvement of lung function and the incidence of adverse reactions before and after treatment were compared between the three groups. Results The FEV1, FEV1/FVC and PEF values of the three groups were higher than those before treatment (P < 0.05), but the difference of FEV1, FEV1/FVC and PEF before and after treatment in the combined nebulization group and the combined systemic group were significantly higher than that of the control group(P < 0.05). PaO2 was significantly increased in the three groups after treatment, and PaCO2 level was significantly lower than that before treatment (P < 0.05). PaO2 difference and PaCO2 difference in combined nebulization group and the combined systemic group were significantly higher than the control group (P < 0.05). The number of vomiting, hypokalemia, abnormal blood sugar and abdominal discomfort in the combined systemic group was significantly higher than that in the control group and the combined atomization group (P < 0.05). Conclusion Azithromycin combined with glucocorticoids can effectively improve lung function in patients with AECOPD. Inhaled corticosteroids are consistent with systemic glucocorticoids in improving lung function in patients with AECOPD, but nebulization can reduce the incidence of adverse reactions.
