      [摘要] 目的 研究 Khorana 联合 Wells DVT 模型预测中心静脉置管相关静脉血栓发生的效能。 方法 回顾性分析庆云县人民医院 2014 年 1 月 1 日~ 2018 年 1 月 1 日行中心静脉置管的 150 例肺癌术后患者,根据是否发生血栓分为血栓组(18 例)和无血栓组(132 例);通过 t 检验检测两种模型变量在不同组间的差异;受试者工作特征曲线(ROC)评估其预测血栓发生的效能;按照不同模型危险状况分组,通过 Log-rank检验分析不同危险组间总存活时间(OS)和无进展生存时间(PFS)的差异。 结果 根据血栓发生分组,血栓的发生与高血脂有关(χ2=4.006,P=0.045),在不同年龄、性别、病理类型、是否患有高血压、糖尿病无关(P > 0.05);两组间 t 检验结果显示:Khorana 模型和 Wells DVT 模型累积得分在不同组间差异均有统计学意义(P < 0.05)。ROC 曲线分析两种模型联合预测血栓发生曲线下的面积分别为 0.812,敏感性 80.4%,特异性 79.3%,Youden 指数> 0.305。Log-rank 检验提示:按照两种模型血栓危险度分组,中低危组 OS 均优于高危组(P < 0.05),而 PFS 未见统计学差异(P > 0.05)。 结论 Khorana 联合 Wells DVT 模型预测血栓发生效能优于单一模型,值得临床进一步推广应用。

      [Abstract] Objective To study the value of Khorana combined with Wells DVT model for predicting the risk of central venous catheterization-related thrombosis. Methods One hundred and fifty patients with postoperative lung cancer who underwent PICC from January 1,2014 to January 1,2018 in Qingyun People’s Hospital were retrospectively analyzed,all them were divided into thrombosis group(18 cases)and non-thrombus group(132 cases)according to whether thrombosis occurred.And t test was used to detect the difference between the two model variables.Receiver operating characteristic curve(ROC) was used to evaluate the efficacy of predicting thrombosis.Grouping according to different model risk status,overall survival(OS) and progression-free survival(PFS) were analyzed by Log-rank test among different risk groups. Results According to the occurrence of thrombosis,the occurrence of thrombosis was associated with hyperlipidemia(χ2=4.006,P=0.045),and it was not related to different ages,genders,pathological types,hypertension,and diabetes (P > 0.05).The t-test results showed that the cumulative scores of the Khorana model and the Wells DVT model were statistically different between the different groups (P < 0.05).ROC curve analysis to analyze the combined prediction of thrombosis by two models showed that the area under the curve of thrombosis was 0.812,sensitivity was 80.4%,specificity was 79.3%,Youden index was > 0.305 respectively.Log-rank test showed that according to the two models of thrombosis risk,the OS in the low-risk group was better than the high-risk group(P < 0.05),but there was no statistical difference in PFS(P > 0.05). Conclusion Khorana combined with Wells DVT model is superior to single model in predicting thrombus generation,which is worthy of further clinical application.
